Baastrup Syndrome



There is interspinous bursa fluid (yellow arrow) and bilateral facet joints effusion (white arrows). An associated right posterolateral facet joint synovial cyst is noted (yellow circle), with mass-effect on the adjacent thecal sac



Key Points

  • Contact of adjacent spinous processes (kissing spine), with resultant reactive changes that may cause back pain.

  • MRI can reveal interspinous bursa fluid and an associated posterocentral epidural cysts.

  • Can be treated by conservative or surgical means.
